We are mindful that registering at a new practice can be daunting and that there can be confusion as all surgeries have their own way of doing things. Please read this quick guide for information that is specific to how we operate at Stoke Gifford and Conygre Road Medical Centres.
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Registering with the practice. If you wish to register you may print off a registration pack, fill out all the forms and bring them all in to the practice. Please also check with the practice before hand that you are within our practice area. We will be unable to register you if you live outside the practice area.
In addition to these forms you will need to bring along with you 2 pieces of identificaton. One will need to be a piece of photo identification (for example a driving licence or passport) and, preferably, something with your address on (for example a utility bill or official letter). Please let us know if cannot provide these documents and our patient services team will support you with your registration.
You will need to fill in one registration pack per person.
